Forms for you to download and complete

• Patient/Client Information - This form is completed on your first visit to South Forsyth Animal Hospital. This is a multi-page form that includes your contact information and information about your pets.
• Surgery Consent Form - This form is used whenever a pet is put under anesthesia. This includes dental procedures.
• Internet Pharmacy Policy - Clients who request written prescriptions in order to order drugs off the internet are asked to read and sign this policy statement. The signed form will be kept in your pet's folder for future reference.
• Diabetic Drop Off - Clients with diabetic pets need to complete this form when they bring their pet to SFAH.
• Authorization to Sedate - This form is required whenever your pet needs to be sedated in order to perform medical procedures or bathing/grooming.
